CDHD’s Community Health Worker program was created to help connect people with the health services they need. We’ve expanded the program to help those in Hall, Hamilton and Merrick counties receive access to services, regardless of income, history or language barriers. We have expanded the Community Health Worker program in recent months in order to meet the increased needs of our district.
Our Community Health Workers come from a variety of backgrounds and experiences to better serve our area. Whether it’s new mothers, newcomers to our community or customers newly on the healthcare market, Community Health Workers can connect people with services. They are also out in our community and doing the hard work of health education.
