 CHI Health hospitals will award $1.2 million in 2025 Community Health Improvement Grants ranging from $5,000- $50,000. 

What you need to know:

· Application portal opens June 17- August 16, 2024.

· Applicants must be a 501(c)3 nonprofit or have a fiscal agent that is.

· Applications must address one or more significant health needs in the hospital’s most recent community health needs assessment (CHNA) or implementation strategy.

· Applications are for one year of funding, beginning March 2025.

· Current 2024 Community Health Improvement Grant awardees are eligible to re-apply for funding.

· Hospitals will release either a small (under $20k) or large grant ($20k+) funding opportunity. Large grants will be rounded to $10k increments ($10k, $20k, $30k and $50k awards available). 

· Large grants require identified collaborating partners.
