Community Health Workers

A CHW is a trusted member of our community. They are able to build trusting relationships and link individuals with the systems of care in the communities they serve. Our CHW's also build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. Here at CDHD we have CHW's from diverse backgrounds speaking a variety of languages including Spanish, Arabic, and Somali . . .

Collective Impact

The focus of Collective Impact at Central District Health Department is community engagement through partnerships with local agencies that work collaboratively to ensure the health of our community. Our team provides individuals and families access to resources through Central Navigation. We believe the health of the community starts at home with families. Rooted in Relationships

Rooted in Relationships is a statewide program that partners with communities to implement evidence-based practices that enhance the social-emotional development of children, birth through age 8. A recent report detailed the results of this program  for Hall County youth.
